The Black Coalition for AIDS Prevention (Black CAP) is a non-profit, community-based organization that serves Toronto’s diverse African, Caribbean and Black (ACB) communities. The mission of the agency is to reduce the transmission of HIV within Toronto’s Black communities while enhancing the quality of life of those living with or affected by HIV/AIDS. Our approach to services recognizes that holistic approaches are required as we work with people who are living with, or at risk for, HIV. Black CAP is an inclusive space that provides services to a diverse ACB population that includes newcomers, LGBTQ communities, youth, and other at-risk populations. POSTION SUMMARY

Under the general direction of the Director for Health Promotions, The ACB Trans Masculine HIV Testing Navigator will assist the MSM HIV Testing Project Coordinator with program implementation. The navigator will facilitate HIV Testing, Referral and Counseling (TRC) and prevention educational services to the African, Caribbean, Black Trans Masculine (ACBTM) community.  The navigator will develop and implement HIV prevention, health promotion and community development initiatives targeting ACB Trans Masculine individuals in Ontario and create supportive environments for those who are HIV+ and/or at risk of infection.

KEY RESPONSIBILITIES

HIV Testing Outreach & Community Development = 60%

  • Collaborates with project staff in advertising and media promotion of the HIV testing program.
  • Responsible for coordinating HIV testing on-site and through outreach into the community, including other community settings where the target population socializes and/or congregate as needed.
  • Conduct street and community outreach to target populations at various venues such as support groups and agency presentations.
  • Assist in coordinating online outreach efforts, including social media management, content creation, blog posts, email blasts, etc.
  • Attend community events to provide HIV testing Outreach.
  • Manage and coordinate the delivery of workshops and drop-in support to ACBTM participants in areas related to sexual health, harm reduction, substance use, mental health and their relationship to risk for HIV.
  • Ensures on-going support to ACBTM by maintaining contact by way of in person drop-in.
  • Attends required agency meetings and external meetings with other local providers.
  • Assists in documenting program activities, and reports outcomes as required.
  • Performs all other duties, as assigned and necessary for operations.

 

HIV Testing Navigation Services = 20%

  • Develops PrEP/PEP resources available in the area for the target population in need of the services.
  • Develops a network with sexual health services and PrEP/PEP providers.
  • Provides access to prevention services (PrEP/PEP), ensuring access, follow-up, and adherence to PrEP/PEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance.
  • Provides and ensures access and active referrals to the screening of STIs.


HIV Linkage to Care = 20%

  • Performs follow-up risk assessments with individuals who test negative for HIV to reinforce the individual’s realistic perception of risk.
  • Collaborates with HIV Testing provider for linkage, and re-linkage to prevention services, STIs screening, and follow up.
  • Maintains communication between inter, and intra-agency services providers for successful coordination of services.
  • Maintains records of all activities.

 

QUALIFICATIONS

  • Experience working within an anti-oppressive and cultural competent framework.
  • Ability to reach out to and connect with ACB Trans and Non-Binary communities.
  • Holistic understanding of HIV/AIDS, STIs and hepatitis C, sexual health, substance use and harm reduction, sexualized violence and homophobia/heterosexism/transphobia prevention, and anti-oppression work, population health, and health promotion strategies.
  • Excellent written and verbal communication skills.
  • Strong experience in public speaking and group facilitation.
  • Strong project management skills, including: working independently, time management, organizational skills, and knowledge of evaluation tools.
  • Strong community development skills and ability to promote and sustain healthy community partnerships.
  • Demonstrated ability to work co-operatively and collaboratively with volunteers, co-workers and community partners.
  • Strong computer skills, including relational databases, word processing, desktop design, web management.
